Two New Funding Opportunities
- Cooperative agreements: Funding for Tribes, Tribal Organizations, and Urban Indian Organizations to support the development of models of comprehensive and sustainable dementia care and services. Apply by June 27, 2023.
- Program awards: Funding for IHS Service Units working in partnership with Direct Service Tribes that provide applicants two options: Comprehensive Model of Care awards and New Care and Services awards to address sustainable dementia care and services. Apply by June 12, 2023.
Eligibility: Tribes, Tribal Organizations, and Urban Indian Organizations. To be eligible for this funding opportunity, an applicant cannot be an existing awardee under the IHS Addressing Dementia in Indian Country program.

Funding Amount: An estimated $1,200,000 per year in total funding is available, pending the availability of funding and IHS priorities.

Purpose
The purpose of this program is to support the development of models of comprehensive and sustainable dementia care and services in Tribal and Urban Indian communities that are responsive to the needs of persons living with dementia and their caregivers. Awardees will:
Awardees will:
- Plan and implement a comprehensive approach to care and services for persons living with dementia and their caregivers that addresses five key areas.
- Develop, in collaboration with the IHS Alzheimer's Grant Program, best and promising practices to include tools, resources, reports, and presentations accessible to Federal, Tribal, and Urban health programs as they plan and implement their own programs.
- Identify and implement reimbursement and funding streams that will support service delivery and facilitate sustainability.

Eligibility: IHS Service Units working in partnership with Direct Service Tribes. Applicants must be able to provide ambulatory care services directly.
- Option 1: Comprehensive Model of Care awards - IHS Service Units providing primary care direct services to Tribes; Tribes that are not currently receiving an IHS grant under the Addressing Alzheimer’s in Indian Country Grants program are eligible. In the instance of IHS Service Units serving more than one Tribe or Tribal Organization, refer to additional eligibility requirements in the Request for Applications.
- Option 2: New Care and Services awards - All IHS Service Units providing primary care direct services to Tribes.
Comprehensive Model of Care Funding: Up to $200,000 per year for each of two years to support the development and implementation of culturally-appropriate comprehensive and sustainable dementia care and services, pending the availability of funding and IHS priorities.
New Care and Services Program Award Funding: Up to $50,000 per year for up to two years to support the development and implementation or expansion of specific services for persons living with dementia or the caregivers of PLWD, including necessary training, education, or collaboration with multi- disciplinary teams or programs.
Funding Amount: Awards of between $50,000 - $200,000
- Comprehensive Model of Care Grantees may apply for a maximum of $200,000 per year.
- New Care and Services program awardees may apply for a maximum of $50,000 per year, and
Total Period of Performance Length: 2 years
Budget Period Length: 12 months
