Models of Care Grants

Cherokee Nation staff

In 2021, the Indian Health Service received funding to support American Indian and Alaska Native communities affected by Alzheimer’s disease and other types of dementia. Through the Alzheimer’s Program, we use these funds to help tribal and urban Indian clinics, health systems, and communities better meet the needs of individuals with both undiagnosed and diagnosed dementia, as well as their caregivers.

The Alzheimer’s Program Models of Care Grants:

  • Are guided by consultations with tribal and urban communities.[PDF - 276 KB]
  • Provide funding to tribes, tribal organizations, urban Indian organizations, and IHS direct service facilities.
  • Support the development of culturally appropriate models of care that effectively address the unique needs of these populations.

Since our first funding opportunity Exit Disclaimer: You Are Leaving www.ihs.gov  in 2022, we have awarded over $7 million in grants to enhance Alzheimer’s care and support within these communities.
