Behavioral Health Reporting Portal
The Behavioral Health Reporting Portal is the online reporting system where funded projects submit their annual progress reports.
Annual progress reports are due once a year. The Portal opens on November 1st and reports are due by November 30th, 2021.
After entering their password on the Behavioral Health Reporting System (BHRS) login page, grantees must select "BHRS" from the dropdown menu to start and complete the Annual Progress Report. If you have any questions, please email LCDR Monique Richards, Zero Suicide Program official.

Grantees ONLY: When the Annual Progress Report is completed, print a copy of the report from the Behavioral Health Portal as a PDF file and upload the PDF report file to your GrantSolutions account.
IHS Federal Awardees ONLY: IHS Federal awardees are required to submit the Annual Progress Report electronically in the Behavioral Health Reporting Portal .
Quarterly Federal Financial Form (SF-425)
The SF-425 is a standard financial form that projects must use to report cumulative expenses incurred for each grant for Grantees and for each program award for Federal Awardees. Projects are required to submit a quarterly Federal Financial Form (SF-425).

Grantees ONLY: Grantees are required to submit the quarterly SF-425 to Payment Management System (PMS) and to GrantSolutions.
Federal Awardees ONLY: Federal awardees are required to submit a copy of the quarterly SF-425 to the Behavioral Health Reporting Portal.
User names and passwords are required to access the Behavioral Health portal and the GrantSolutions system.
