Gallup Indian Medical Center Screening Eligibility Criteria for Early COVID-19 Therapy

All agents are authorized for patients who are 65+ or who have at least one clinical condition that confers high risk for sequelae of COVID.

Therapies
- Paxlovid oral therapy
- Symptom onset within 5 days (onset date = day 0) with no new hypoxia
- Evaluate for drug-drug interactions, Check Liverpool COVID-19 Drug Interaction Checker (Google it);
- Check renal function and renally dose as follows:
- eGFR >60 mL/min, no adjustment necessary
- eGFR 30 to 60 mL/min, reduce to 150 mg nirmatrelvir and 100 mg ritonavir BID x5 days
- Do not prescribe if:
- Severe hepatic impairment (Child-Pugh Class C)
- eGFR <30 mL/min or on dialysis
- Short Course Remdesivir (200 mg on day one followed by 100 mg on days two and three, infused in 30-120 mins)
- symptom onset within 7 days (onset date = day 0)
- creatinine clearance >30ml/min in the past 7 days, if no labs available can get CMP and coags at first infusion visit (coags done at baseline due to uncommon SE of increase in PT)
- commonly given to inpatients who have early COVID and are admitted for a non-COVID reason
- Bebtelovimab infusion
- Symptom onset within 7 days (onset date = day 0) with no new hypoxia
- Only authorized for use in patients for whom no other FDA authorized therapy is appropriate or accessible
