BACKGROUND

This transmittal forwards Amendment No. 2 to Part 3, Chapter 14 of the Indian Health Manual.  The amendment contains provision for establishing the School Health Record and the School Health Record Control Card.

CHANGES

  1. Number first seven pages of Chapter 14 as page 1, page 2, etc.
  2. Remove newly numbered pages 5, 6, and 7 and substitute the enclosed pages 5, 6, 7, 8, and 9.
  3. Number remaining pages as page 10, page 11, etc. through page 13.
  4. Substitute the revised Table of Contents to this chapter and discard that transmitted by TN 7.
