What is the Personal Health Record?
The Indian Health Service’s Personal Health Record (PHR) is a convenient, on-line portal used by IHS patients nationwide to access their health records, or those of a loved one. PHR allows patients to track their appointments, their medications, and their medical test results from the privacy of a personal computer or mobile device. It also allows patients to share their health data with designated personal representatives.
Another key feature of PHR is the secure messaging system. This system allows patients to communicate with their health care providers safely and securely.
Who can use the Personal Health Record?
Patients who qualify for health care under the Indian Health Service and their authorized personal representatives can register to use PHR.

Register in Four Simple Steps:
- Create your PHR account at https://phr.ihs.gov/phr/PHRRegistration/InitPHRRegistration1
- Visit your local Indian Health System Facility
- Bring one form of identification with you (see examples below)
- Show identification to the PHR Registration Clerk at the Indian Health System facility
