- Description: Present findings from the Strong Heart Study--a large epidemiological study of cardiovascular diseases and its risk factors in 12 American Indian communities in the USA. Much of the presentation will highlight diet, physical activity, and other potentially modifiable risk factors. We will also outline major barriers and facilitators to healthy diet quality, and present findings from the Cooking for Heath Study--a National Heart, Lung, and Blood Association-funded intervention focused on improving diet quality and diabetes outcomes among American Indians with diabetes from the North-Central USA.

- Description: An Indian Health Service (IHS) National Health Coach Pilot Project (NHCPP) is underway providing health coach training to practitioner and health professionals to understand how to integrate health and wellness coaching into the IHS Tribal, Federal and Urban health care systems. Results from the NHCPP will provide information on program development and health care integration. This webinar will provide an overview of the state of health coaching in healthcare, describe the purpose and background of the NHCPP, and share how health coaching can be incorporated into diabetes programs to meet the updated 2023 ADA Standards of Care in Diabetes around supporting positive health behaviors.

Nutrition psychology is the study of the role nutrition plays in mental health. Long-term stressors and a poor diet can cause a depletion of nutrients that support neurotransmitters vital for feelings of well-being. Ongoing inflammatory processes have been shown to exacerbate depression and the anti-inflammatory diet is explored in research as an intervention. An anti-inflammatory diet is also ideal to prevent long-term complications of diabetes and therefore, can be a part of the diet plan to help patients improve mood and motivation for a better quality of life. The dietitian-nutritionist plays an important role in supporting patients in their mental health within their scope of the nutrition assessment, diagnosis, and intervention.

- Description: Primary care case management includes activities that integrate additional support to help patients achieve goals within primary care and/or coordinate care needs that are dependent upon services beyond those offered in primary care (e.g.; specialty care, community services). Health coaching is a model of chronic disease health education that can be readily integrated into team-based primary care. Health coaching occurs in a coach-patient relationship, providing tailored, culturally appropriate education and self-management support. In this presentation we present two Chinle IHS programs which health coaches utilize case management to extend their help of patients. For the past 11 years, we provided diabetes education in primary care clinics using bilingual, bicultural health coaches. The coaches target higher risk
patients, including individuals with new onset diabetes, poor glycemic control, or insulin use. The health coaches connect
patients to other clinical services (e.g.; Medical Nutritional Therapy, Native Medicine). For a selected group of patients, the health coaches provide additional case management services to help patients maintain continuity. With the support of the Diabetes Program, a second program of case management with health coaching was developed, focusing on patients with complex care needs.
