The Indian Health Service Division of Grants Management is issuing a reminder to all applicants and grantees that they are required to submit applications and other documents using the appropriate Office of Management and Budget (OMB) forms.

Please note that if additional space is required beyond what the official form provides, you may attached additional documents when submitting the OMB form.
