At the Northern Navajo Medical Center in Shiprock, New Mexico, we have always envisioned a health care model that includes delivering care directly to patients who are experiencing homelessness. Homeless individuals often have difficulty coming in to appointments and are seen as high-utilizers of emergency department services.
To address this issue, in October 2021, our Health Promotion and Disease Prevention team launched Sih Hasin, a street medicine mobile health care program for those experiencing homelessness. This is the first program of its kind, serving rural communities on a reservation. Through our mobile clinic, we offer a variety of services that include physical exams, wound care, and treatment of acute and chronic medical conditions. All services offered have a similar or the same standard of care as they would when offered in a traditional hospital setting. The clinic also offers COVID-19 testing and vaccines.
