BACKGROUND:
The purpose of this Transmittal Notice (TN) is to transmit Part 3, Chapter 3, "Health Information Management," Indian Health Manual (IHM). This revised chapter updates the Indian Health Service (IHS) health record services policy, objectives, staff responsibilities, operating relationships, and standards. It is IHS policy that all health information professionals, health care providers, managers, and staff who are responsible for the creation, maintenance and disposition of health records, shall maintain and preserve the confidentiality of the patient's health record. The chapter formally adopts the name change from "Medical Records" to "Health Information Management." The revised chapter implements an IHS electronic health record system, integrates the regulatory requirements of the Health Insurance Portability and Accountability Act, and complies with the Centers for Medicare and Medicaid Services regulations and other laws.

MATERIAL SUPERSEDED:
Part 3, Chapter 3, "Medical Records," IHM, Manual Appendix 3-3-A through 3-3-C, and the Table of Contents
MANUAL MAINTENANCE:
Remove the superseded chapter in its entirety and insert the attached Part 3, Chapter 3, "Health Information Management," IHM, including the Chapter Table of Contents, in its place after Part 3, Chapter 2.
Log and file this TN in sequential order.
