Fort Yates Hospital
The Fort Yates Hospital, located near the Missouri River on the Standing Rock Sioux Reservation at Fort Yates, North Dakota, is a fully accredited 12 bed hospital staffed by Physicians and Nurse Practitioners. The hospital provides comprehensive primary care based on the Primary Care Medical Home (PCMH) model. The Emergency Department offers 24/7 care and is designated as a Level IV trauma center. Additional services include inpatient care, outpatient care, dental services, behavior health, optometry, nutritional counseling and telehealth.

Health stations are located in the Standing Rock Sioux Reservation communities of Cannonball (ND), Bullhead (SD), and Wakpala (SD). These health stations provide primary care staffed by Nurse Practitioners, Registered Nurses and Community Health Representatives. The health stations are visited at least monthly.
